DIAGNOSIS:

2. Usual interstitial pneumonia (UIP) with adenocarcinoma

There is a classic usual interstitial pneumonia pattern with subpleural basal predominant honeycombing and traction bronchiectasis with no other finding. This, in a smoker, is highly suggestive of idiopathic pulmonary fibrosis (IPF).

The nodule in the right upper lobe shows irregular margins and spiculation and is worrisome for neoplasm. The incidence of malignancy in patients with IPF is more than in the normal population. This was biopsied and turned out to be adenocarcinoma.
