DIAGNOSIS:

4. Hypersensitivity pneumonitis

The images show a fibrosing ILD with an axial distribution pattern. There is subpleural sparing and there is no zonal predominance. Widespread patchy lucencies due to areas of air trapping are seen.

These findings are characteristic of chronic fibrotic hypersensitivity pneumonitis.
