This is a 36-years old lady who presented with right sided radiculopathy. The MRI shows a classic lesion with a central target sign on T2W images, suggesting a neurogenic tumor. The first image is a STIR coronal, followed by a T2W axial, then T2W sagittal and then post-contrast T1W axial and coronal images.
The patient and doctor wanted biopsy confirmation. It is best to go through the sacral bone, like the trans-sternal biopsy and the trans-sacral biopsy I discussed last week.
Biopsy showed a benign peripheral nerve sheath tumor. Interestingly, we started the procedure without sedation. The moment I touched the lesion, the patient had pain, so we temporarily stopped the procedure and gave her intravenous sedation and then completed the procedure.
