56-years old lady presented with backache. MRI shows infectious spondylitis at L5/S1. The CT scan shows similar findings. While in India, tuberculosis is commonest infection to present in this manner, other infections can also behave similarly. Hence they need to be biopsied.

Her culture showed Burkholderia infection.
Always biopsy!!!
