This 78-years old man presented with a prevascular space mass in the midline and to the right. He did not have myasthenia graves, but the most likely diagnosis was a thymic tumor.

The biopsy was easy, done with an 18G needle and showed a thymoma (WHO type B3). While there are other differentials to a pre-vascular space mass, such as germ cell tumor and lymphoma, in this age group, a lobulated mass such as this is almost certainly of thymic origin, with the assumption that a mediastinal thyroid extension has been ruled out.
