This is a 52-years old man treated for carcinoma rectum. A PET/CT showed lung nodules.
The CT scan showed that the largest, most accessible nodule measured 5.13 x 5.26 mm.
The patient’s free-breathing was shallow and under control. In the right lateral decubitus position, it was possible to biopsy the nodule successfully using a 20G coaxial biopsy gun with a 10 mm throw. There was a 20 cc pneumothorax that remained stable after 1 hour.

The diagnosis was metastasis.

Small lung nodules can be successfully biopsied especially when peripheral. The shortest route should be calculated. There is no reason to avoid these procedures.
